Timeline / Our History

Primary Care Medical Services of Poinciana, Inc. (D.B.A. Osceola Community Health Services) have served the Osceola County population since October 2004. Since then, our network of community health centers have become an integral part of our community's health care safety net.

Our service area includes the entirety of Osceola County and our target population is the disadvantaged residents who are within 200% federal poverty level (FPL). These residents typically are uninsured or underinsured and have significant barriers to affordable and accessible primary care services. They also are among the most likely to suffer from social determinants of health that lead to health disparities and poor health outcomes.

Our network of ten health centers, strategically located to serve our target population has enabled our most vulnerable citizens to have access to health centers. Our commitment to quality health care was demonstrated by earning The Joint Commission for Ambulatory Care Accreditation and Primary Care Medical Home certification.
